I, as the parent/guardian of the above name (student), do hereby authorize the Medical Staff of Alabama Children's Ministry to consent to any examination, x-ray, anesthetic, medical or surgical diagnosis or treatment and hospital care which is rendered under the supervision of any physician or surgeon licensed hospital, whether such diagnosis or treatment is rendered at the office of said physician or at said hospital.
Further, as parent/guardian of the student above, I do hereby expressly consent that the above student may receive emergency medical treatment from any physician, hospital, or other medical center without the necessity of approval in case of the emergency contact is unreachable, and do further agree to hold blameless any physician, hospital, or other medical center for rendering such services nor the United Pentecostal Church International of Alabama and the Medical Staff of Alabama Children's ministry.

I am confirming and give permission for the above student to attend Alabama Crusader's Camp. I also confirm that all the above information is correct. I understand that Alabama Children's Ministry and Staff are not in any way/form liable nor can be held responsible for accidents or injuries or financial obligations to or caused by my student. I further promise that above student will follow all guidelines and understand there will be no tolerance of any violation.

I confirm that the mentioned student has been given permission by their called Pastor to attend Alabama Crusader Camp 2024. I understand that the Executive Staff may contact their pastor for verification, issues, and/or behavioral concerns.

I hereby give permission to the UPCI of Alabama and Children's Ministry to use photographs and/or videos of my child/student in printed materials, social media outlets and/or web-site platforms for promotional purposes. I further understand that these images will not be given away, sold, or otherwise disbursed other than for promotional purposes with the UPCI of Alabama and Children's Ministry.
